One important source you can use to weed out the worst facilities from the best is medicare. Government officials routinely inspect local nursing homes, either to investigate a complaint or to confirm that the facility meets US standards.
The Centers for Medicare and Medicaid Services evaluates nursing homes in Ohio and rates each one based on health inspections, staffing and quality measures, such as the number of residents in pain or flu shots per day
Here is a breakdown of the qualifying criteria:
Health inspections such as compliance with health care and medicaid regulations, internal and external complaints, infection control and prevention standards.
Staffing levels, staff turnover, hours worked and hours spent on residents each day.
Quality measures such as flu shots or other vaccines per period of care, recovery rates from surgery, and overall nursing home performance.

You can search the database by location or search for specific facilities by name. In addition to seeing the most recent inspections and nursing home rankings, Medicare identifies all nursing homes cited for abuse.
Of the 68 local nursing homes evaluated, seven were cited for abuse. “Abuse is defined as the deliberate infliction of injury, unreasonable confinement, intimidation, or punishment with physical harm, pain, or mental anguish,” according to Medicare.
